Welcome to our New Patients!     The purpose of this page is to provide you with information to get you ready for care in our office. Below, you will find a Microsoft Word file and a PDF file to download that contains forms to fill out before you come to the office. Please download, print, and fill them out completely.    If you don't have a word processor program that will open Word files, you can download a great Microsoft Office work-alike (word processor, spreadsheet, relational database, slide presenter and art programs included)  open source freeware program called Libreoffice by going to www.libreoffice.org/download Libreoffice will also open PDF files if you don't have Adobe Acrobat.
